United States: The United States Centres for Disease Control and Prevention (CDC) has issued an urgent Health Alert Network notification, addressing both medical professionals and the general public regarding the sustained threat posed by dengue virus infections. Current epidemiological assessments indicate that incidence rates remain elevated within certain US territories, with concurrent surges manifesting in various international regions, predominantly across the Americas.

Escalating Dengue Crisis in US Territories

Puerto Rico continues to grapple with an officially declared dengue emergency, instituted in March 2024, as infection figures have soared by a staggering 113 percent in comparison to the same timeframe the previous year. Similarly, the US Virgin Islands, which formally recognized an outbreak in August 2024, persists in reporting cases, with 30 autochthonous infections documented within the current calendar year, according to CIDRAP News.

Dengue’s Global Proliferation and Domestic Implications

Over the past half-decade, the global burden of dengue has escalated substantially, with the Americas witnessing record-setting prevalence. This epidemiological pattern has precipitated an unprecedented number of travel-related dengue cases within the United States throughout 2024, reflecting an 84 percent increase relative to the preceding year. Notably, three US states recorded instances of locally acquired dengue transmission in 2023, prompting the CDC to caution that vector-borne proliferation within the continental United States may intensify in regions harboring competent mosquito vectors.

According to the CDC, seasonal travel patterns during spring and summer coincide with dengue’s peak transmission period in numerous endemic nations. This temporal overlap heightens the probability of both imported and autochthonous cases within the United States.

Emerging Trends in Dengue Serotypes Among Travelers

All four dengue serotypes were detected among US travelers in 2024, yet epidemiological data reveal a notable surge in dengue serotype 4 (DENV-4) cases in recent months. Consequently, the CDC has advised healthcare professionals to utilize its DENV-1-2 real-time reverse transcriptase polymerase chain reaction (RT-PCR) diagnostic assay when clinical suspicion for dengue is high. Additionally, practitioners are encouraged to employ an updated investigatory framework designed to facilitate comprehensive case analyses through meticulous review of medical records, as per reports by CIDRAP News.

As dengue continues to exert a formidable public health burden, both within the United States and globally, heightened vigilance, rapid diagnostic measures, and strategic intervention remain paramount in mitigating transmission risks.
